Be prepared for your DOT exam
On the day of your exam you will need:

Valid photo ID (driver's license or passport)

Lists of medications/supplements including their strength, dosage and prescribing physician name

Eye glasses, contact lenses (if applicable)
-
Hearing aids (if applicable)
-
Drink water (You will need to provide a urine sample.)

Medical records (see below)
In order to be prepared for your exam please put together a list of any medical conditions you have, the medications and supplements you take, all doctors you have seen, and any medical devices you use.
How is a DOT exam different?
A Department of Transportation (DOT) physical exam MUST be completed by a Certified Medical Examiner (ME). A Certified ME has specialized training provided by the Federal Motor Carrier Safety Administration (FMCSA) and has taken and passed a certification exam. This training must be repeated every 5-10 years.
