Be prepared for your DOT exam

On the day of your exam you will need:

  • Valid photo ID (driver's license or passport)

  • Lists of medications/supplements including their strength, dosage and prescribing physician name

  • Eye glasses, contact lenses (if applicable)

  • Hearing aids (if applicable)

  • Drink water (You will need to provide a urine sample.)

  • Medical records (see below)

In order to be prepared for your exam please put together a list of any medical conditions you have, the medications and supplements you take, all doctors you have seen, and any medical devices you use. 

How is a DOT exam different?

A Department of Transportation (DOT) physical exam MUST be completed by a Certified Medical Examiner (ME). A Certified ME has specialized training provided by the Federal Motor Carrier Safety Administration (FMCSA) and has taken and passed a certification exam. This training must be repeated every 5-10 years.

*Please note: If you have ANY of the listed medical conditions you MUST bring additional medical records with you or your certification WILL be delayed*

Hypertension: Please bring a log of your last 3 months of blood pressure readings. If you see a cardiologist, we may need their records as well.

Neurological or Brain Conditions: We may require a neurologist letter releasing you to work. Include stability of the condition, confirmation that treatment is adequate, safe, and effective, and that driving and operating a CMV is safe.

Lung/Breathing Disorders: We may require copies of pulmonary function test or a letter from the treating physician. 

If you have sleep apnea or use a CPAP, you must have 90 days of data showing proper use (70% compliance). A letter from the  treating physician may also be required

Insulin-Treated Diabetes: You must have an Insulin-Treated Diabetes Assessment form completed by the treating physician.

Non-Insulin-Treated Diabetes: You must have 3 months of downloaded data from a blood glucose monitor (NOT a handwritten log) and a copy of your most recent HgbA1C test. A letter from your treating physician may also be required.

Hearing Deficiency: If you have had an audiogram done, please bring a copy.

Cardiac Disorders: If you have had any cardiac testing (ECHO, stress test) please bring a copy. A letter from your cardiologist may also be required.

Certain Medications: Some medications require a Driver Medication form (these include medications used to treat ADHD, anxiety, depression, insomnia, narcolepsy, diabetes, psychiatric conditions and others).

3: Paperwork

Once your physical is complete, the Certified Medical Examiner will complete all necessary paperwork and submit it to the FMCSA on the driver’s behalf. The driver will be provided a copy of the exam as well as the original certificate.

 

If your employer would also like a copy, the driver will sign a release of information form that gives the Medical Examiner permission to release this information. All records will be kept on file for 3 years.

2: Physical Exam

Generally, DOT exams take 30 to 90 minutes, depending on your medical history. The Certified Medical Examiner will review your health history with you and may ask additional questions as needed. You will also be asked to provide a urine sample. This checks for protein, blood, and sugar in your urine.  (This is NOT a drug screen.)

 

The physical exam will consist of the following:

  • Height and weight

  • Blood pressure, pulse, and oxygen

  • Vision test

  • Hearing test

  • Assessment of muscle strength, range of motion, reflexes, and sensation

  • Complete exam of heart, lungs, abdomen, limbs, and neurological system

1: Medical History

You will be required to fill out a medical history form that provides a general idea of your personal health history and family history including past and present illnesses or diseases.
